Bristol is contracted with most SNF’s/LTC’s & ALF’s in the Colorado Springs area. We have a central intake team who assists us with ensuring quick turn around time and helping with a seamless transition for patients and families during this process. Please contact one of our Liaison’s for any new inquiries
and more information on our specialty programming Bright Moments, Sweet Dreams, Perfect Harmony, and high intensity CNA’ visits. We most recently had a deficiency free survey!
